Registering with the Practice

When you register with the practice you can express a preference of doctor.  Please tell the receptionist.  All newly registered patients over 5 years old are expected to arrange an appointment to have a simple medical check before registration with the practice can be accepted.

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Registration Process

You will need to bring two forms of identification as follows:

Foreign Nationals not Born in this country:  Valid passport and visa plus proof of address which can be a utility bill or bank statement

British Citizens born in this country: proof of ID - driving licence or passport plus proof of address which can be a utility bill or bank statement

Once the above criteria has been satisfied you will be given a registration pack and an appointment for a new patient medical.  Please bring the completed forms to this appointment when you will be registered with the practice.  You will not be registered until you attend for this appointment.

If you are on any medication you will also need to make an appointment with a doctor and bring your medication to this appointment.
