Please give at least 2 full working days notice. Repeat prescriptions may be requested by using one of the following methods:

• Return your order slip to the surgery by post. You can enclose a
  stamped addressed envelope if you would like us to return it to you.
• If you are ordering a large number of items and the envelope will be bulky please provide an envelope measuring 9 inches x 6 inches.  This will still be covered by first class postage.
• Fax your order to us on 01604 646208.
• Bring your prescription request into the surgery and post it in the box at reception.
• Fill in the form below  - You will need to provide the exact details of the medication you require when using this email service. This service is for prescription requests only and no other query will be dealt with.

Please note: for all emails, fax and written requests for repeat medication, please make sure you give your patient ID number (if known), name and date of birth as well as the medication you require.

If you would like Woodview Pharmacy to order, collect and dispense your prescription on your behalf they can be contacted on 01604 670619 between the hours of 10am and 11am Monday to Thursday. PLEASE ALLOW 1 WEEK FOR PROCESSING.

The prescription may be collected from reception or, if requested at the time of ordering, we can arrange for the items to be made ready for collection at Woodview Pharmacy. PLEASE ALLOW EXTRA TIME FOR THE ITEMS TO BE DISPENSED BY THE PHARMACY.
