2 - Complete a New Patient Application and GMS 1 form, either print these or collect from the surgery

3 - Attend surgery in person with photo ID and proof of address dated within 3 months of your current address.

4 - To avoid delay with repeat medication please provide an up to date list of current medication and doses for the Clinician to review

Woodview Medical Centre welcomes new patients who reside permanently within the practice area boundaries.

Please use the web tool provided here to check if your postcode falls within our practice boundary.

British Citizens born in this country:

For each applicant applying to register you will need to provide the following:

  • Proof of identity (e.g. Valid passport, photo driving licence or birth certificate (or for new babies their 'red book')) 
  • Proof of address (e.g. Recent utility bill or bank statement less than three months old or official letter such as a mortgage or tenancy agreement)
     
Foreign Nationals not born in this country:

For each applicant applying to register you will need to provide the following:

  • Valid passport and visa
  • Proof of address  (e.g. Recent utility bill, at least three months recent bank statements or official letter such as a mortgage or tenancy agreement)
Please note : If you are not from an EU country you will need to have lived in the UK for 3 months otherwise you will be treated as a private patient until you have fulfilled the residency requirements.

Once the above criteria has been satisfied you will be given a registration pack with forms to complete. If you are on any regular medication you will need to make an appointment with a doctor or nurse practitioner and bring your medication to this appointment,
 

You will need to complete the following forms:

  • A GMS1 Form (or your current medical card)
  • An Alcohol Screening Test
  • A Declaration Form
  • An immunisation history form (only for all children aged 5 and under)

 You can pick up a registration pack from reception.

Please ensure you bring a complete registration and GSM1 form, Proof of photo ID and Address to the surgery. The patient must be present at time of registration.
